A sign-off checklist template for work going live

Most launch mistakes come from work that was assumed to be checked. A sign-off step with named signers makes the check visible.

The sign-off block

  • Item: [the campaign, page, report, change or send being signed off].
  • Version: [the file, link or change ID that was reviewed].
  • Checked by: [the person who ran the checks, not the person who made the change].
  • Client approval: [name, date and channel, or 'not required' with the reason].
  • Checks passed: [each check from the checklist as yes, no or not applicable].
  • Known gaps: [anything not checked, and why].
  • Go live time: [date and time in the client's time zone].
  • Read-back: [who checked the live result, and when].

Rules

  • The maker never signs alone. A second person confirms.
  • If one check fails, the item does not go live. Fix it, then sign again from the start.
  • A sign-off that is not recorded did not happen.
Made-up scenario: a new landing page is ready on Thursday. The maker signs the checks, the account lead signs on Friday morning after the client approves by email, and the read-back that afternoon confirms the page and its tracking tag are live.
