An access request form template for ad accounts and tools

Access given in a hurry is rarely taken back. A form with an end date makes every grant a decision that someone approved in writing.

The form

  • Requested by: [name, role and agency].
  • Client and account: [the exact ad account, store or tool, with its ID].
  • Level requested: [read, draft or apply, and why no higher level is needed].
  • Reason: [one sentence on the task this grant serves].

  • Approved by: [the client owner, with the date and the channel used].
  • Logged by: [who entered the grant in the register].

Rules

  • Grant the lowest level that does the task.
  • Give each person their own grant. Never share a login.
  • Remove the grant on the end date, and note the removal in the register.
Made-up scenario: Dana, a made-up analyst, asks for draft level on a client's ad account for two weeks to prepare a test. The client owner approves by email on Monday. The grant ends on the fourteenth day and the removal is logged.
Use this form with the MCP permissions guide when the grant is for an AI connector.
